Photographically, the contrasting background with a line running right through his head pulls my viewing eye away from the subject. The border effect draws attention to the border, and makes me realize it's really close to the subject.
Despite these things that a photographer might see, the subject and his family may indeed love this photo. It puts him in a positive light, and he appears vibrant and full of happiness for life.
