The Black Rapid FastenR-3 is the point of contact between your camera and your R-Strap. The R-Strap is worn diagonally across the torso from shoulder to hip and is secured to the camera via the FastenR-3 - successor to the FastenR and FastenR-2 -, which in turn screws into the tripod socket.
